Understanding Hidradenitis Suppurativa: Symptoms, Causes, and Treatment Options

If you’ve ever dealt with painful bumps under your skin that seem to come back over and over again, you may have wondered if something more is going on. For many people living with Hidradenitis Suppurativa (HS), that’s exactly how the condition starts

— a painful lump that looks like a boil or ingrown hair, heals for a bit, and then suddenly returns.*

HS is a chronic inflammatory skin condition that affects areas of the body where skin rubs together, like the underarms, groin, inner thighs, under the breasts, and buttocks. It often begins after puberty and is most commonly diagnosed in adults under 40. While anyone can develop HS, studies show it appears more frequently in Black individuals, which may point to a genetic connection.

What makes HS especially difficult is that it affects much more than your skin. It can impact confidence, relationships, mental health, and even day-to-day comfort. Many people spend years trying to manage symptoms on their own before realizing they are dealing with a medical condition that deserves treatment and support.

The Early Signs of HS Are Easy to Miss

One reason HS is often overlooked is that the early symptoms can seem minor at first. A lot of people assume they are dealing with razor bumps, acne, cysts, or irritation from sweating and friction.

Usually, the first sign is tenderness beneath the skin before a lump even appears. Then comes a deep, painful bump that may feel warm, swollen, or sensitive to touch. Some people only get one at a time, while others notice several forming in the same area.

Over time, those bumps can become larger and more painful. They may connect beneath the skin, drain fluid or blood, and leave behind scarring once they heal. Some people also notice blackhead-like spots that appear in pairs.

These recurring flare-ups are among the most recognizable symptoms of hidradenitis suppurativa. Unlike a typical pimple or ingrown hair, HS tends to recur in the same spots.

HS Can Become Physically and Emotionally Exhausting

People who don’t have HS often underestimate how painful and frustrating it can be.

Imagine trying to walk, exercise, sit comfortably, or even wear certain clothing while dealing with painful nodules in sensitive areas. During flare-ups, even simple movement can become uncomfortable. Some people describe the pain as constant pressure under the skin, while others struggle more with drainage, irritation, and tenderness.

Then there’s the emotional side of it.

Many people with HS feel embarrassed about the appearance of flare-ups or worry about odor and drainage. Some avoid social situations, dating, the gym, or even routine activities because they fear discomfort or judgment. Over time, that can seriously affect mental health and self-esteem.

Unfortunately, many people suffer quietly because they feel uncomfortable talking about symptoms that occur in intimate areas of the body. But HS is far more common than many realize, and getting help early can make a major difference.

Is Hidradenitis Suppurativa an Autoimmune Condition?

The short answer is—not exactly —but the immune system is definitely involved.

Researchers are still learning about what causes HS, but current evidence suggests the condition is tied to an overactive inflammatory response in the body. In other words, the immune system may overreact, triggering inflammation around hair follicles and leading to painful flare-ups beneath the skin.

HS is generally considered an autoinflammatory condition rather than a traditional autoimmune disease. Genetics, hormones, smoking, weight, stress, and friction on the skin may all play a role in triggering symptoms or making flare-ups worse.

Hidradenitis Suppurativa Treatments: What Actually Helps?

One of the biggest misconceptions about HS is that people simply need “better hygiene.” That is not true. HS is not caused by being dirty, and scrubbing the skin aggressively can actually make symptoms worse.

The good news is there are several hidradenitis suppurativa treatments that can help manage flare-ups and improve quality of life.

For some people, small lifestyle adjustments can help reduce irritation. Common hidradenitis suppurativa remedies include:

  • Wearing loose-fitting clothing
  • Using gentle cleansers on affected skin
  • Applying warm compresses during flare-ups
  • Managing weight to reduce skin friction
  • Avoiding smoking
  • Identifying foods that may worsen inflammation

Dermatologists may also recommend prescription treatments depending on the severity of symptoms. These can include topical antibiotics like clindamycin, oral anti-inflammatory medications, hormonal therapies, or biologic medications that target inflammation deeper in the body.

In more advanced cases, procedures or surgery may be necessary to drain painful abscesses or remove damaged tissue.

You Don’t Have to “Just Live With It”

One of the hardest parts about HS is that many people normalize their symptoms for years before seeking help. They assume recurring boils are simply something they have to deal with.

But ongoing pain, drainage, scarring, and flare-ups are not something you should ignore.
